for taking Application & Registration for
“A” and "B" Category |

Make
a DD of Rs. 1000/- for A-Category and Rs.1250/-
for B-Category towards application &
registration fees from any Bank in the favour
of “Dr.
VRK Womens' Medical College” |
- CCarry the Original EAMCET-11 H.T and Rank Card, Intermediate Pass Memo, Latest T.C. and show it at the Counter. Submit the DD and One Set of Xerox of all Certificates to obtain the Application Form.

Fill the Application Form legibly and enclose
the following Self-Attested certificates |
i. |
Copy of EAMCET-11 Hall Ticket & Rank Card |
ii. |
Copy of Intermediate/ 12th Exam Memo |
iii. |
Copy of SSC/ Equivalent Exam Memo |
iv. |
Latest TC/ Migration Certificate |
v. |
Copy of the Passport/Driving License / PAN Card/Voter ID as proof of residence of Parents. |
vi. |
Copy of Passport /Driving License/ PAN Card/ Ration Card/Voter ID of references signed/ attested by them
respectively. |
vii. |
Two Latest Photos with candidate’s signature at the back. |
viii. |
Bonafide & Character Certificate from the last Institution to be enclosed, which would mention the status of his/her behavioral pattern specially in terms as to whether he/she has displayed persistent violent or aggressive behavior or any desire to harm others. (This is required as per letter No. MCI-34 (1)/2009-Med/5191 to 5401 dtd. 30-4-2009). If in case, this is not available at the time of registration, then it must be submitted at
the time of admission |

On
submission of Filled-in application along with
all the enclosures, Registration Slip must be
collected.
No separate registration charges
are to be paid. |
- If you are submitting the downloaded form in person then the form number will be issued along with the registration slip.
- If you are sending the downloaded form/ Original Form by post make sure it shall be delivered before cutoff time.
- If submitting the downloaded form by email, then the payment can be made by credit card . All the enclosures shall be scanned and attached.
This procedure is common for both ‘A’ category and ‘B’ Category. |
The candidate can also submit the Downloaded Form or Original Form through email by scanning all the sides of the form alongnwith all the enclosures in a zipped file. The name of the Zipped file shall be the A-EAMCET HT.No: or B-Name-of-Candidate for A & B Categories respectively Candidates submitting the downloaded form through email have to pay cost of form through Credit Card facility provided in this website’s relevant page.
